Record of Training Hours

All workshops are approved for CEHs* and many are approved for DCS Foster Parent hours. In order to document attendance and hours, please download the fillable PDF below. For each workshop you attend, note the workshop name, the hours (default is 1.5), and the workshop code (given by the workshop chair at the end of the workshop). Additionally, if you watched the workshop through our on-demand option, you'll need to also include your quiz score.

You can complete the form by typing into the PDF itself or by printing it out and handwriting your information.

Once you have documented all of the workshops for which you'd like training hours, please save or scan the document and email it to registration@iarca.org. You'll receive a confirmation of receipt. Please save that confirmation for your records.

NOTE: Please make sure to save the form to your computer BEFORE entering your information. Otherwise, you will lose the data you enter into the form.
